Our Patient Experience Concierge Live Hours are Monday through Thursday from 9am to 5pm and Friday from 9am to 2pm. (We are open by appointment only on Saturdays from 10am to 2pm, however, our Patient Experience Concierge Team is only available Monday-Friday to book appointments). For appointment requests that match our availability, we will go ahead and book your appointment and notify you via email that the appointment was booked. If we do not have availability on the days and times that you have requested, we will reach out to you to find an alternate appointment time. We kindly ask that you give us at least 24-hour notice for appointment cancellations or rescheduling.

COSMETIC INJECTABLE APPOINTMENTS

For NEW patients scheduling either a cosmetic injectable consult or a Botox/Dysport treatment for the first time with Trisha Kennedy Roman, MSN, FNP-BC, NP-C - Please CLICK HERE to complete a new patient questionnaire at least 24 hours prior to your scheduled appointment. Please note that dermal filler injections or Sculptra injections cannot be performed the same day as the consultation. A treatment plan will be outlined and then the procedure(s) will be scheduled following the consult. Consults can include same day treatments for Botox or Dysport treatments after a treatment plan is discussed.

For EXISTING INJECTABLE PATIENTS who have already had dermal filler treatments with Trisha or who have already discussed a treatment plan and know what area you are having treated, we can go ahead and schedule the filler treatment for you. You will have to complete the injectable appointment and cancellation policy by CLICKING HERE within 24 hours of scheduling the appointment in order to hold the appointment time. If you have already prepaid for your filler treatment, CLICK HERE for this form instead. Again, this form must be completed within 24 hours of scheduling your appointment in order to hold your appointment time.

All Sculptra treatments require a consultation prior. If you have already discussed a treatment plan with Trisha at a previous appointment and wish to go ahead and schedule the procedure, we can go ahead and schedule the Sculptra treatment for you. You will have to complete the injectable appointment and cancellation policy by CLICKING HERE within 24 hours of scheduling the appointment in order to hold the appointment time. If you have already prepaid for your Sculptra treatment, CLICK HERE for this form instead. Again, this form must be completed within 24 hours of scheduling your appointment in order to hold your appointment time.

For EXISTING PATIENTS who have never had dermal fillers or are not certain of what area of the face they want to have treated, a consult is required first and then a separate treatment date will be scheduled following the consultation.

**Consultations are complimentary; however, they are still taking up a significant amount of time on a high demand schedule, so if you are unable to make your appointment, we kindly ask that you give us 24-hour cancellation notice.
